Hannah Tolar, CRNP

Hannah Tolar is a board-certified Family Nurse Practitioner with Southern Clinic who is dedicated to providing compassionate, patient-centered care to individuals and families throughout the Wiregrass region.

A native of the Wiregrass area, Hannah earned her Associate Degree in Nursing from Galen College of Nursing in Tampa, Florida, followed by her Bachelor of Science in Nursing and Master of Science in Nursing with a Family Nurse Practitioner specialization from Chamberlain University. She is board certified by the American Academy of Nurse Practitioners.

Prior to becoming a nurse practitioner, Hannah gained extensive experience in critical care, cardiovascular intensive care, cardiac catheterization, cardiovascular recovery, and home health nursing. She has special interests in chronic disease management, preventive care, geriatric medicine, and wellness.

In addition to her clinical practice, Hannah serves on the Alabama Board of Assisted Living Administrators and on the Tribal Council for the Southeastern Mvskoke Nation in Troy, Alabama.

When she is not caring for patients, Hannah enjoys spending time with her husband and two children at the lake, the beach, and traveling.
